After a stay at a skilled healthcare facility such as a hospital, rehabilitation center, or nursing home; home health care services can prevent you from returning by reinforcing the measures put in place and translating their implementation in the home. A trip to the hospital can address the immediate issues at hand, but only home health care can help change the habits and behaviors that may have caused the issues, and implement changes to prevent them from occurring again.

Surgical procedures, falls/incidents, strokes and other traumatic events can be life altering, and in some cases, these events can cause a patient to be temporarily, or permanently homebound. In those cases, our nurses, therapists, and home health aides are ready to serve you, often within 24 to 48 hours after a referral is made an assessment can be completed. What do we do?

Skilled Nursing

Registered and Licensed Nurses provide overall management of patients medical needs, which includes educating patients and their caregivers about medications, diet, and healthy living.

Home Health Aide

Certified home care aides assist patients with activities of daily living including bathing and grooming, working with and under the supervision of Registered Nurses and Therapists.

Physical Therapy

Physical Therapists assist patients rebuild their strength and endurance following an illness or surgery. Physical Therapy helps the patients to rebuild strength, balance, and endurance.

Occupational Therapy

Occupational Therapists help patients regain independence in their activities of daily living, including dressing, grooming, and eating. It is especially helpful during recovery after strokes.

Speech Language Therapy

Speech Therapists are trained to help patients with improvement of speech and treatment of swallowing disorders. Our Speech Therapists are especially helpful during recovery from a stroke or other neurological problems.

Medical Social Worker

Board-certified social workers help patients by assessing their needs and evaluating their qualifications for available community resources.
